Yup… Looks like it’s a left hand on the right side. It’s almost like there’s an invisible person standing shoulder-to-shoulder to him holding a cigarette to his mouth.

Don’t worry about the hand. It only goes to show what smoking does for you. The idea is great and this is so well done. A belated welcome to this website.
